If you wish to inquire about coverage and are concerned about medical history, you may fill out the confidential request form, but allow 2-3 days for us to shop rates for you.
Most insurance carriers require blood work to be done, if you wish to apply for the most competitive rates. On the request form, please let us know what the most convenient time for testing would be. We will notify a national paramedical service and they will confirm the time and location. There is no charge for this examination. The paramed will witness your signature on the application and send everything back to us.
